Who Should Respond?; Penalty For Breaching Sanitary Rules; Reactivation Of Advocacy; Deaths In Hospital Rooms; Happy Father’s Day

There is a lot of hate and one-sided opinions in the world today. One of the newest crazes is to defund the police, seemingly everywhere. I think back to the police who care for my small town of Antigo, Wisconsin and everything I’ve seen there, they’ve done a fantastic job. Maybe it’s because it’s a small town? Maybe they had better training? I’m sure they’ve made mistakes before.

Once, as a teenager coming home from work, I was dropping some aprons off to my mother’s house as she was going to make some slight repairs (I was living with my dad at the time). Anyways, I got stopped at 3AM on a weekend in the summer. I was driving a 1977 Chevy Monza Spider Edition. It was a great “first car”; that little 8-cylinder beast had me passing everything on the road except for gas stations though. The officer ran my plates, walked over to my car and then asked me, “Any idea why your license plates come back belonging to a MONTE CARLO?”. “I assure you sir, I’m just as confused as you are.”, was the only thing I could think of saying. He was a rooky. He seemed nervous. I think he was on the border of “I may have a stolen vehicle” or “I may have screwed up somehow.”. He told me to turn off my engine, but I had already turned it off. He was definitely nervous. It was a wretched standoff. You could feel the tension with every passing second. Who was going to act first? Would he go for his gun? Should I start my car and speed off? Or maybe I could just ditch that $1,000 gas-sucking piece of crap, make a run for it and just live the rest of my life as a fugitive. As time passed, I knew it was him or me. My index finger impatiently tapped the steering wheel. I felt that first bead of sweat across my brow. That was it, I was going to make my move. I knew these neighborhoods better than anyone and I was like a rabbit when I was on the move. I knew that once I cut through Mrs Arrowoods yard I would get to the factory behind it and I was scot free. This rooky cop son-of-a-bitch didn’t have a chance. I reached for my switchblade knife, sliced my hand and used the blood to paint dark red streaks across my face as I jumped from the car yelling, “A Mikkelson will never be taken alive!!!” LOL. Nope, I was NOT going to do that. Actually, I was going to sit my ass in that car and wait for “officer newbie” to come back and eventually tell me, “Sorry sir. I saw the abbreviation “MON” and thought that meant Monte Carlo. Here’s your license sir, it’s late, drive safely, watch for deer, have a good night.”

It’s uncanny to me how people think that they should run, fight, attack, insult, be stupid, etc., especially if they have nothing to hide. I went everywhere in that car and I drove fast almost all the time. I would get pulled over more times than I should because of my own damn fault 99% of the time and when I did,

  1. I would ask for a break and then
  2. Ask for a lighter fine and then
  3. Ask if I could pay with check in the name of the “State of Wisconsin” on the spot and just get it over with.

I wasn’t going to fight it there and I wasn’t going to fight it in court. I messed up and I would pay the consequences (if I couldn’t get off with a warning). I even got pulled over once by one of my Boy Scout Leaders. Man, talk about embarrassing.

The rooky cop that pulled me over that one tense and conflictive summer evening.

That being said, I totally agree that police should not always be the first responders for everything. Don’t get me wrong, I do not believe in defunding them completely; that would be ludicrous. The majority of the “disturbances” could be handled by other, “first responders” or “councilors”. It would be an amazing social test to see how most people will react. I don’t think they should go completely unarmed, but perhaps something with less of a chance of lethality. Taser. Pepper spray. Feathers to tickle them with. Whatever, I don’t care. What would happen? The real thugs will attack and take advantage of the more submissive first responders and the police would come to save THEM. Eventually, this new first responder team will refuse to go to certain areas or will request to also be armed with something more “compelling”. The police will end up in charge of certain areas that are considered impossible to manage passively. The first responders will take the more commonly-known peaceful sectors. Also, they should allow citizens to dictate what service they prefer. Each home will sign up for either police response or the “good-hearted taser-toting pacifist social councilors”. The homes that sign up for non-police intervention, would automatically have a secondary police response team on call as a precaution. Hey, what the hell, let’s give it try! If it doesn’t work, we revert back to the more professional police system with more stringent accountability.

Please sir, if you’d kindly get off of me, perhaps we could have an amicable discussion of how we could assist you with proper counseling and further government-funded assistance.

After the investigation that the Metropolitan Health Region opened this morning, after the meeting held yesterday by the PRD Caucus to choose their proposal for the board of directors of the National Assembly (AN), in a restaurant in the town , the deputy Mayín Correa reacted to said decision, since this call was signed by Minister Rosario Turner.

Correa on his twitter account shows that the call was scheduled to take place on June 18 at 2:00 pm, in the Ever Green Building in San Francisco, an event that was postponed until the afternoon, since the deputies were in extraordinary sessions, sitting the Moratorium Law in its third debate.

The Secretary General of the Party, Pedro Miguel González explained that the convocation was made in said restaurant considering that as it was a partisan activity, it could not be held in the facilities of the National Assembly, and the party headquarters is in a process of remodeling.

However, González clarified that what was hired was a space for the meeting and not as a restaurant, ensuring that all biosafety regulations were met.

The party representative apologized to the citizens and to the owners of the establishment for the inconveniences, at the same time that he asked the Health authorities to carry out the corresponding investigations in order to clarify what happened.

‘We request the health authorities to carry out the corresponding investigations in order to clarify what happened, assuming the consequences that may arise, reiterating that our sole purpose was to comply with a constitutional, legal and statutory mandate, and not circumvent the rules imposed to guarantee the health and well-being of Panamanians, ‘he said in the statement.

COULD YOU POSTPONE THE OPENING OF BLOCK 3 OF ECONOMIC ACTIVITIES?

It is still unknown whether block three of activities could open next week, in view of the situation in the country with COVID-19.

According to Dr. Julio Sandoval, in two or four weeks the installed capacity of hospitals will be very compromised if the number of cases of COVID-19 continues to increase at the rate it is going. So the opening of Block 3 in his opinion should not be so soon.

Gladys Guerrero Epidemiologist from the Ministry of Health (Minsa) agrees with Sandoval, that an in-depth evaluation must be made before making the decision to open another economic block.

Block 3 covers retail and wholesale trade, car sales, construction in the private sector, and professional and administrative services.

Dr. Marcel Penna Franco of the group of specialist doctors at the Santo Tomás Hospital is concerned that the increase in cases of COVID-19 will lead to hospitals to collapse and that doctors will be left without the personal protective equipment required to care for patients with the virus. On the subject, businessman Rubén Castillo believes that an alternative could be to segment the blocks that are going to open.

“That not all companies open at once, but that little by little they are activating, but the direct message is that the blocks must be opened so that more people can receive their salaries and state aid goes to the most vulnerable sectors” Castillo noted.

While the head of the metropolitan health region, one of the most affected by COVID-19 infections, warns that everything will depend on the behavior of the virus and populations, whether to open or postpone it.

She also said that if things continue to be aggravated by social indiscipline, they could even evaluate closing the second block.

In declarations given by the head of Epidemiology of the Minsa, Lourdes Moreno, with the opening of block two, an increase in hospitalizations in the ward and a slight increase in intensive care were perceived.

Until now the effective transmission rate of COVID-19 in Panama stands at 1.58 and the virus doubling time has been reduced to 16 days.

DEATHS IN THE ROOM, A TREND FOR SEVEN DAYS

Where do most of the patients who battle Covid-19 die: in the ward, intensive care (ICU), at home or in emergency rooms? This is one of the questions many people ask who closely follow the behavior of the new coronavirus.

Although the health authorities have not presented a detailed report that clearly answers the question, for seven days in June they specified in their daily reports the specific place where the deaths were recorded.

In those seven days there were 59 deaths from Covid-19, of which 38 occurred in hospital wards, 15 in intensive care, 3 in emergency rooms and 3 in homes.

Reynaldo Chandler, a pulmonologist and part of the advisory team of the Ministry of Health, said that although patients come to the wards in a mild (stable) and moderate state (alteration of their vital signs), many present risk factors, such as age ( over 65) or chronic diseases.

Another factor is that, on average, patients are admitted between the fifth and sixth day of contracting the virus, with advanced disease, which does not allow immediate mobilization to the ICU.

“When they get to the hospital, they go so badly that even though the doctors intervene, they fail to resuscitate the patient. They die between the first 48 and 72 hours after arriving at the hospital, ” he concluded.

Julio Osorio, an intensivist doctor and secretary of the National Negotiating Medical Commission, explained that only serious patients with specific criteria are admitted to the ICU, such as those with a prognosis for survival and urging intubation. “The norm is that every year more people die in hospitals than in the ICU, and the same thing happens with Covid,” he said.

Early diagnosis, key against the virus

The director of the Metropolitan Health Region, Israel Cedeño said yesterday: the coronavirus has a high lethality, especially in people with risk factors. There are people who are diagnosed as positive on a Monday, without any symptoms, but in a very short time, between 24 hours and 48 hours later, they become serious patients.

Cedeño used this example to report how the virus behaves with regard to deaths, and said that precisely because of that behavior, they hold workshops in communities to detect the disease as soon as possible.

In fact, specialists agree that several days anticipating the serious stage of the disease can be decisive in saving the lives of those infected.

The figures of the Ministry of Health (Minsa) give a clearer idea. Of the 470 deaths reported in the country until last Wednesday, 12.5% (59, in total) occurred in a 7-day period, from June 10 to 17 – not including 14 -. And, of those 59 deaths, 38 occurred in hospital rooms, 15 in intensive care units, 3 in emergency rooms and the same number in private residences.

Doctors speak

Reynaldo Chandler, a pulmonologist and member of the Minsa advisory team, explained why more patients die in hospital wards than in intensive care. For example, he stressed that patients with Covid-19 arrive in the wards in mild and moderate states, but often up to five and six days after being infected and with the disease very advanced. They even enter with acute respiratory failure.

“There is an idea that most of the patients with Covid-19 die in the ICU, but the reality is that most of them die in the ward. These are statistics being analyzed, ” said Chandler.

He also explained that, by hospital rule, patients who have a low prognosis for survival are cared for in wards and not in the ICU. “This has always been the case in hospitals,” he said.

In addition, he specified that there are many more patients in the wards and, therefore, the death rate is also higher. In fact, most of the hospital beds are in the ward: 4,989, versus 458 in the ICU.

According to the specialist, the variable “time” is important to guarantee the life of the patient, because when they arrive at the hospital with the virus in its most aggressive phase, despite the fact that the doctors intervene, they cannot resuscitate the patient.

Precisely, he agreed with Cedeño that they die between the first 48 and 72 hours after arriving at the hospital.

For this reason, he considered it “valuable” that the person take the test on time.

Other statistics

An epidemiological report by Minsa details that the first case was announced in Panama, from March 9 to June 7, there were 393 deaths from Covid-19.

72.3% of these deaths correspond to people older than 60 years, being the highest mortality rate in the group of 80 years.

Regarding gender, 67.9% (267 of 393) were men and 32% (126 of 393) were women. The relationship between the sexes in general is two deceased men for each woman.

Meanwhile, 77% of deaths occurred in the most complex hospitals, such as the Dr. Arnulfo Arias Madrid Hospital Complex of the Social Security Fund (CSS) and the Santo Tomás Hospital.

4.3% of deaths have occurred in homes.

Julio Osorio, intensivist doctor and secretary of the National Negotiating Medical Commission, explained that intensive care is only entered by patients with very specific criteria, such as those who are serious but have a prognosis for survival, as well as those who need mechanical ventilation to breathe.

Osorio, who is head of the intensive care unit of the Rafael Hernández Hospital in Chiriquí, explained that in any hospital in the country there will always be more deaths in wards, because it houses more patients. “If we see that the person does not have probabilities, she remains in the room,” she concluded.

Minsa was asked for the full number of deaths in hospital wards and intensive care, but there was no response.

PHARMACEUTICAL ASPEN ACCELERATES PRODUCTION OF DEXAMETASONE

South African pharmaceutical company Aspen Pharmacare said on Friday it was ready to “speed up” its production of dexamethasone, a powerful steroid, effective in treating the most seriously ill of COVID-19.

“We are able to accelerate production if demand requires it,” one of the managers of the group that markets it, Dr. Stavros Nicolaou, told AFP.

“We are always evaluating what could be the world demand” for this medicine, he added, “and clearly if it increased a lot we would have difficulties.”

Those responsible for the British clinical trial Recovery announced Tuesday that dexamethasone reduced deaths by a third among patients under artificial ventilation, and by a fifth those who were less severely affected, with oxygen or intubated.

Dexamethasone is manufactured in the form of tablets at a plant in Aspen, Germany, and also in the form of injections produced in various countries through trade agreements.

WITHOUT SYMPTOMS OF COVID-19, IMMUNITY MAY BE WEAKER, ACCORDING TO STUDY

People infected with the new coronavirus who did not develop symptoms of the disease may have a weaker immune response against the virus than those who did fall sicker, Chinese researchers suggest in the journal Nature Medicine.

The researchers warn, therefore, about the risks of implementing “immunity passports” that would guarantee their holders to be protected against evil.

The study describes in the immunological and clinical plan the case of 37 people without symptoms whose infection was diagnosed by a nasopharyngeal virological test in the Wanzhou district (in the municipality of Chongqing, southwest China) before April 10, 2020. .

Among those 37 asymptomatic patients identified in a group of 178 people infected with the new coronavirus, 22 were women and 15 men, aged 8 to 75 years (mean age: 41 years).

The authors, Ai-Long Hua, from Chongqing Medical University and their colleagues, found that these patients, isolated in hospital, had an average duration of viral excretion of 19 days, compared to 14 days in 37 patients who presented symptoms ( fever, cough, breathing difficulties).

This duration of excretion does not mean, however, that they infect, which remains to be evaluated, the authors reveal.

Eight weeks after discharge from hospital, levels of neutralizing antibodies, which a priori give immunity to the virus, decreased 81.1% in patients without symptoms, compared to 62.2% in patients with symptoms.

To further explain the immune response, the researchers measured some substances (cytokines and chemokines) in the blood and observed their low levels in asymptomatic patients, showing a reduced anti-inflammatory response.

These data, as well as previous analyzes of neutralizing antibodies, highlight the potential risks of using “immunity passports”, and therefore advocate the continued application of common preventive health measures.

HOW MUCH DOES THE GOVERNMENT PAY FOR EACH ROOM IN HOTELS FOR COVID-19 PATIENTS?

The increase in cases of people with coronavirus, led the Government to have to take alternatives to try to control the spread of this disease, hosting them in hotels that went on to become hospitals.

Currently $ 15 is paid daily for each room, plus basic services such as: water, cable and Internet.

The president of the Panamanian Association of Hotels (Apatel) pointed out that, to date, there are 2,000 rooms available in hospital hotels nationwide and, if more are required, they would make them available.

As of Wednesday, June 17 of this year, 773 patients with COVID-19 had been accommodated, representing the payment of $ 11,595 in one day.

Doubts One of the concerns that comes to light is that, once the reactivation of this economic activity in block 4 occurs, how will visitors (nationals or foreigners) be guaranteed that the hotel will be free of COVID- 19. Rodríguez indicated that this will be the responsibility of the Ministry of Health, who will guarantee the disinfection of hotels that have been used to treat patients with coronavirus.

While Alexis Zapata Romero, president of the Association of Tour Operators, considers that the Government, through the Ministry of Health, must guarantee protocols so that once airports are reopened, tourists will have security when staying.

For now, the majority of COVID-19 patients staying at these hotels are part of the National Police, the National Border Service, and others who have requested support with transfers.

ENDNOTES

Happy Father’s Day.

I say this mostly to me, to you, to everyone who is or who has a father or a father-figure in your life.  I say it to those who miss their father in heaven.  I also say it to that mom out there who took the time to play both roles.  That’s not to ALL single moms, because not all single moms really made the two-role effort.  My mom did.  She did a great job.  The way she played both roles was by getting me closer to my grandfathers even when she didn’t necessarily approve.  She let dad come over whenever he wanted to see me as well.  She never spoke poorly of him, at least not until I was probably 25-28, or something like that.  Not a peep prioreven when her blood was boiling over one thing or another.  He’s not an easy person to get along with.  But Mom kept at it, getting me interested in Scouts along with my cousins, taking me fishing, teaching me about honor and pride.

Then there’s my dad.  My dad is far from perfect.  But who is, really?  I do believe that he did what he thought was right based on his own upbringing, finances and other limitations.  Seldom around, but never out of my head, he was a very small man, but with a HUGE ego.  He’s not your typical Viking (except the “ego” part).  If you knew me, you’d thinkby my size, I came from a line of large Viking men.  However, my size didn’t come from the Mikkelson side of the family, it came from the Wick family.  I’m only 5’11” and 250lbs but Dad is probably 5’7” and 165lbs.  (Grandpa Wick was easily 6’3”, 270lbs.)  But for the record, Dad’s165lbs was 100% mean, feisty, Marine Corp S.O.B.  So, Happy Father’s Day to you too, Dad.
