Big Changes Next Week; Biosecurity Guides for Retail Companies; RT descends in Panama; Fiscal support measures; Dedication to the Fluffies

Friday, August 21, 2020

OK ladies and gents, we have quite a week in front of us.  For months we’ve been waiting for some substantial change to lighten the quarantine and gain a major boost to Panama’s economy.  Well, I’m here to tell you that next week, ain’t that week.  However, we do have some important changes that are still quite notable.

  1. First, to be clear, this weekend, Saturday and Sunday, we are still on full quarantine, which essentially means that only 70% of the cars that are normally on the road during the week, should still be seen on Saturday and maybe half of that still on Sunday.  In other words, no changes yet.
  2. Beginning on Monday, August 24, for Panama and West Panama, there will no longer be a by-cedula number restriction.  Saturdays will also be open for men again.  Ha, who’s the weaker sex NOW, ladies!?!?  So essentially, women can go out anytime between the hours of 5AM and 7PM Monday, Wednesday or Friday.  Men as well during the same hours, Tuesday, Thursday and Saturday.  Although, I think Saturday has curfew starting at 5PM.  I’ll confirm in our next report.  We have time guys, we have time…
  3. Also, on Monday, some businesses are opening to include Real Estate offices, appraisals and wholesale distributors of non-food items, including bookstores.  I love bookstores, but I don’t see how they’re “wholesale”.  Maybe anything more than 10 books at a time?  Who’s with me on that one??  😊
  4. On Tuesday, I should be getting my paint gun through Amazon, will be ordering plywood, cedar and other woods so the following weekend I will officially start building the cabinets for my kitchen.  It’s been crap since the day I moved in and I keep saying, “someday, I’ll build it up”, well, that day is finally here… …almost…  …more like, it’ll be here next week, more or less.

And that about sums up the major changes for the next week.  I’d really like to have a serious coming-to-Jesus with the powers-to-be about when I can go fishing.  “That better be part of the following week’s plan, buster!”, I’d tell Sucre or Cortizo.  I may leave out the exclamation point and possibly the word “buster”.  I may also refrain from being too assertive, demanding or annoying.  Actually, I would probably be groveling and crying, begging for them to let me take me boys fishing.  I know that life isn’t fair, but this takes “not being fair” to a whole new level.

In the meantime, have a great weekend everyone!

Spread of the coronavirus through food packaging is possible, but unlikely

The transmission of the new coronavirus via frozen or packaged food is once again taking center stage along with the outbreaks in China, Germany and New Zealand.

The investigation of the first outbreak that took place in Beijing, China, in mid-June, suggests that the new coronavirus may have been imported in frozen food, after the infection of employees and detection of the virus in kitchen tools.

Likewise, in Germany, at the end of June, 1,553 positive cases of COVID-19 were reported – La Sexta details – linked to workers of a meat company in Guetersloh, North Rhine-Westphalia.

And, in New Zealand, 102 days after not identifying any local case of infection, a new group of infected could be linked to a frozen storage plant. But, for now, it is ruled out that it is the main route of transmission, Bloomberg collects from statements by the general director of Health Ashley Bloomfield.

Spread of the coronavirus through food packaging is possible, but unlikely.

However, the main concern is around an unanswered question that Sarah Cahill, of the Codex Alimentarius Commission – of the World Health Organization and the United Nations – has exposed to Bloomberg: what is detected when SARS-CoV- 2 on these surfaces?

Experts do not believe that contagion through this route is highly probable, although they emphasize that it is possible. And, so far, they cannot be sure if the identified virus is an infectious agent or is no longer viable.

“We know that viruses can generally survive if they are frozen. That means that, in theory, it is possible for the infection to spread that way,” Benjamin Cowling, director of epidemiology and biostatistics at the University of Hong Kong, tells Bloomberg.

For its part, despite the fact that the WHO estimates that almost 1 in 10 people in the world becomes ill after eating contaminated food, the Centers for Disease Control and Prevention (CDC) rule out that there is evidence of this transmission in the spread of the new coronavirus.

“There is no evidence to support the transmission of COVID-19 associated with food,” they emphasize.

Meanwhile, the Organization of Consumers and Users (OCU) in Spain ensures that there is no known contagion of coronavirus through ingested food.

These are based on the fact that SARS-CoV-2 has very little ability to survive on surfaces and has to survive several processes: freezing, thawing, contact with a human and the probability of reaching the contagion routes.

These are usually the nose and mouth, but they can also be the eyes, which are 100 times more contagious in this coronavirus than SARS from 2002, according to a study published in The Lancet.

MINSA ISSUES GUIDELINES FOR COMPANIES THAT ARE DEDICATED TO RETAIL SALES IN PANDEMIC

The Ministry of Health (Minsa) published this Thursday, August 20 in the Official Gazette a resolution approving the health guide for retail sale via home or by vehicle without contact with the customer.

The Guide establishes as a requirement the creation of a special health and hygiene committee for the prevention and care of COVID-19 in each company.

Businesses must determine their maximum capacity of people for each closed area of the company, establishing a security perimeter of meters between people.

Avoid employing vulnerable people such as those over 60 years of age, workers with comorbidity and pregnant women.

In these cases it is recommended to use teleworking. In addition, good ventilation is required to be maintained in dispatch areas and other work areas.

The guide also instructs frequent hand washing, use of a mask, physical distancing, and the commitments that the company must assume with its workers.

Among these commitments are guaranteeing access to basic supplies for hand hygiene such as liquid soap, paper towels and alcoholic gel; provide personal protective equipment; and have supplies for cleaning and disinfection of surfaces, among others.

Businesses will have to constantly monitor for signs and symptoms. For this, the temperature of the employees will be taken at the beginning of their working day. If high temperatures occur, you should immediately contact line 160 and inform the health authorities.

Failure to comply with this guide will lead to the application of sanctions by the corresponding authorities.

PANAMA MUST BE REGISTERED IN COVAX TO GET EQUAL ACCESS TO THE COVID-19 VACCINE, FEDEFARMA

Currently there are two companies that have their vaccine proposals in phase 3 and that could be ready by the end of the year.

The executive director of Fedefarma, Victoria Brenes, explained that the Covax mechanism is the pillar of the program called “Vaccine Accelerator”, which brings together the pharmaceutical industries and international health organizations, and through which cost-effective access will be allowed. equitable vaccination for all countries of the world.

Against this background, Brenes said that in order for Panama to have access to a reasonable cost to the vaccine that is approved to combat COVID-19, it must be registered and after the authorization of the vaccine, it will have a period to pay the payment of 15% of the total cost of this.

Of course, Brenes pointed out that in the first delivery phase each country will be supplied with 20% of its population, with which it will seek to immunize the population at risk, first.

It should be noted that Fedefarma represents the innovative pharmaceutical industry that investigates, develops and markets drugs and therapies that prevent, treat and cure diseases; and in it are registered about 19 laboratories that operate in Central America and the Caribbean.

Brenes stressed that according to the latest report from the World Health Organization (WHO) there are 29 vaccines under clinical evaluation of these 7 are in phase 3 and there are two of the companies that are more advanced in their phase three, they estimate that by the end of year could have the approval on the part of the scientific authorities for its use. In addition, there are another 140 vaccines that are in the preclinical phase.

On the other hand, Brenes added that “the pharmaceutical industry is working together at an incomparable speed, without sparing resources to promote the movement not only of a potential vaccine, but of treatments.”

DOCTORS CLAIM PAYMENTS DUE; DO NOT DISCARD A STRIKE

Doctors are on the front lines of the battle against Covid-19.

“We are hundreds of professionals who were hired in times of Covid-19 and we demand fair treatment.” This is how Dr. Michelle Chávez has expressed herself, who denounced that she, like many of her colleagues in the health sector, have not received payments for their fortnights.

“We are not asking that they give us anything. We want payment for the days we have worked, where we expose our lives, many times without the necessary protective equipment,” said Chávez, a general practitioner in the hospital’s Covid-19 wards. Nicolás A. Solano de La Chorrera.

Chávez said that many colleagues have three-month employment contracts and that they may not have to wait until after six months to get paid. She also complained that, as she said, they were made to sign a labor contract “without defining the funds to make the payments.”

“We have shown that we are capable and that we have the vocation, because we have continued working,” said Chávez, who began working last June at the Chorrerano hospital and has not yet received the corresponding payments.

Chávez said that they are demanding a negotiation table so that the authorities commit to pay them all the debt on August 31. She warned that if we do not “have an answer by August 31, next September 1, we could enter into a sit-down strike. We cannot live on applause ”.

For his part, the Minister of Health, Luis Francisco Sucre, announced that this Thursday, August 20, an extraordinary session will be requested in the Budget Commission of the National Assembly. He assured that they already have the economic resources, but that they have to transfer items. Sucre affirmed that these payments must be disbursed in the next few days. “We are doing everything possible to accelerate this process,” he said.

Through a statement, the Ministry of Health has reported that this entity is aware “that the processes that by law must be followed to cancel these payments have not been carried out as quickly as we would have liked, however, we reiterate that, at the as soon as possible, payments will be canceled in full. “

LOOKING FOR VOLUNTEERS FOR VACCINE TRIALS AGAINST COVID-19 IN PANAMA

The call for Panamanians who want to be part of the team of volunteers who participate in vaccine trials has already started. Citizens must be between 18 to 60 years old.

In social networks there is already the flyer that explains about this call for vaccine trials, there would be about 250 people.

Dr. Xavier Sáez-Llorens explained who can be candidates for these trials in phase 2: “The mechanics is to sign up on a list, the people on the list are called and an appointment is scheduled, then there they are made medical and nursing evaluations, informed consent is presented, the person is adequately informed about the potential risks, the potential benefits and decides if they want to participate or not. “

“If you want to participate, then you sign the consent with a witness and submit to all the relevant questions and questionnaires of the project and then you are told on that day the vaccination will begin.”

Currently, Panama has been chosen for two studies of vaccines against COVID-19, this one that is in phase two and there is another that is in phase three, but some steps are still pending to be able to start the trial in Panama.

BUBBLE TRACE REVEALS NEW CORONAVIRUS CLUE

Bubble trail reveals new clue about coronavirus.

A doctor examining COVID-19 patients in a coma for signs of a stroke has come across a new clue about how the virus would affect the lungs, thanks to a test that uses tiny air bubbles and a robot.

Dr. Alexandra Reynolds, a neurologist at Mount Sinai Health System in New York, listened in bewilderment to the “cacophony of sounds” of harmless bubbles passing through the bloodstream in patient after patient.

But the strange finding piqued the interest of lung specialists, who are trying to find an explanation for why the most severe coronavirus patients do not get enough oxygen even when they are on a ventilator.

It is a revealing fact of how, after months of the start of the pandemic, scientists have difficulty unraveling the multiple avenues of attack of the coronavirus and find clues in unexpected places.

In May, when the sick flooded New York hospitals, Mount Sinai’s intensive care unit – which usually treats brain problems – was immediately transformed into a COVID-19 ward, in which respirators kept the sick alive. heavily sedated.

“When we wake them up, will we find they have some horrible brain injury?” Reynolds wondered anguished, who at first had no way of monitoring brain function other than looking at the pupils of the sick.

A test called transcranial echo doppler uses sound waves to track blood flow in the brain, but it was too dangerous for staff to stay too long next to the patient’s head.

For this reason, Reynolds resorted to a new robot version, an artifact that is placed on the patient and automatically performs the tracking. With this, she carried out a bubble scan, a painless method to determine if there is a risk of spillage and that consists of injecting physiological solution containing tiny air bubbles into a vein. As the microbubbles circulate, the smallest blood vessels in healthy lungs – called capillaries – filter them out of the bloodstream.

Over the course of several nights in the ICU, Reynolds tested some of his most serious coronavirus patients, and time and again, NovaSignal’s doppler robot measured bubbles that, far from being filtered, reached the brain.

“It was really weird,” Reynolds said. Sometimes the bubbles bypass the filter when passing through a heart defect that is a known risk factor for stroke, but “it was impossible for everyone to suddenly have a hole in their heart.”

But for Mount Sinai lung expert Dr. Hooman Poor, the mystery of the bubble could be the “missing link” in determining why these patients are not getting enough oxygen. Perhaps it was not a heart defect that allowed the bubbles to pass but an abnormal dilation of the lung capillaries.

Poor and Reynolds continued the tests. At the end of the pilot test, 15 of the 18 patients had microbubbles in the brain. Additionally, in support of Poor’s theory, patients with the highest amount of bubbles had the lowest oxygen levels, the researchers reported in the American Journal of Respiratory and Critical Care Medicine.

What is the importance of capillaries in this case?

Coronavirus sufferers on respirators have acute respiratory distress syndrome (ARDS), an inflammation that causes lung failure when other infections harden the lungs by blocking oxygen, but the coronavirus does not harden the lungs in the same way, Poor said.

Doctors know that the coronavirus attacks the lining of blood vessels and causes dangerous clots. According to Poor’s new theory, the bubble study suggests that blood is diverted from obstructed vessels to abnormally dilated ones and thus flows too quickly, preventing it from properly absorbing oxygen.

A rare disorder called hepatopulmonary syndrome causes the same abnormality and is diagnosed by bubble study.

These preliminary findings do not show that the problem is in the dilated blood vessels, but certain autopsies have linked COVID-19 with deformed pulmonary capillaries.

Now a larger study is being carried out to determine if the bubble measurement allows us to know if the patient improves or worsens.

The report is “new proof that the answer is in the blood vessel,” said Dr. Corey Kershaw of the University of Texas, who was not involved in the pilot study.

He said that it is not yet definitively proven that the problem does not lie in a heart defect.

But “it’s an example of the many things we don’t know yet,” said Kershaw, praising the creativity of those who found this clue.

PROBLEMS IN FIGALI HAVE BEEN CORRECTED AND THERE ARE ALREADY HOSPITAL ROOMS AVAILABLE, ACCORDING TO THE MINISTER

The downward trend of the COVID-19 contagion rate in Panama, which remains at 0.99%, is what has allowed the authorities to make certain mobility measures more flexible in the capital and West Panama, explains the Minister of Health, Luis Francisco Sucre.

He said that the trend in decreasing incidence in the number of cases, installed capacity and, of course, the behavior of the population, are some of the factors that give some peace of mind and are elements that allow decisions to be made regarding the opening of new activities.

He reiterated to the population the call to COVID-19 positive people to go to hospital hotels, since it is the best option for both the patient and their relatives, but unfortunately people are a little resistant to answering that call.

According to the minister, the foregoing could be achieved if there were a law or if the Panamanian idiosyncrasy were to heed the recommendations of the health authorities. Currently there is no mechanism to force a patient to go to a hospital hotel.

However, it warns that in the event that a person breaks or breaches the home quarantine and endangers public health, sanctions can be applied.

In fact, the minister said that a decree has been signed, authorizing local and regional authorities and the National Police (PN) to take a person who is found violating health measures.

Complaints for attention in hotels

Some people have complained about the follow-up given to patients who are admitted to hotels, where the staff is not aware of the evolution of the virus.

According to the minister, when these complaints are received, the hotel is investigated and the personnel who are on duty are investigated to take the measures and correct the failures.

What he did clarify to the population is that there is no team to give five-star attention to all the people who are confined in the hotels, since it must be remembered that they are facilities that have been readjusted to be able to care for patients.

Leaks in Figali

The minister clarified that in the facilities of the Figali Convention Center there are already rooms where patients can be received.

He explained that the “unforeseen” with the leaks in the roof were discovered with the intensification of the rains, but the issue has been corrected and “thank God”, there is still the capacity in hospitals to receive patients. He explained that in the Figali Convention Center there are four rooms: three with 40 beds to receive moderate patients; Of these three, there are two that are in total condition and a leak was found in the third.

Payments due

Regarding the doctors who have spoken, unhappy because they have been owed their biweekly payments for months, Sucre affirmed that he understands the situation and hopes that by this day the answers will be received from the National Assembly (AN), since the funds must go through the National Budget Office (Diprena).

From there, approval must be given by the AN’s Budget Commission for what they will request an extraordinary session, in order to make the payments, according to Sucre.

He also regretted the death of people during the pandemic and gave condolences to relatives, as well as the death of a nurse this Wednesday, August 19.

RUSSIA WILL BEGIN OUTDOOR TRIALS OF THE VACCINE THAT THEY’VE ALREADY APPROVED

Russia plans to start international clinical trials of its Sputnik V coronavirus vaccine, which it has already approved for use, while increasing the number of national volunteers to more than 40,000 people starting next week.

Saudi Arabia, the United Arab Emirates and the Philippines will participate in the trials, while Brazil and India are in talks to provide volunteers, said Kirill Dmitriev, head of Russia’s sovereign direct investment fund, which backs the vaccine, without specifying the date. The tests will adhere to international standards at home and abroad, he said.

“This will be one of the largest ongoing vaccine trials in the world,” Dmitriev said. “It will be done in parallel with the vaccination of our high-risk workers, such as health workers.”

Russia faces criticism for its haste in registering and administering the vaccine, which Moscow’s Gamaleya Institute is developing with the wealth fund and the Defense Ministry, and for a lack of transparency with its data.

A “major publication in the West” will deal with the Russian scientific article, Dmitriev said. The expert declined to reveal the name of the outlet because of “political pressure” concerns.

President Vladimir Putin this month touted the vaccine as the world’s first after Russia granted it an emergency registration before clinical trials were completed. Russia has the fourth-highest number of cases globally and, although the initial peak of infections has passed, it still reports an average of more than 5,000 new cases per day in August.

The name of the vaccine, Sputnik V, refers to the launch into space of the first artificial satellite in history, which the Soviet Union carried out in 1957.

More than 160 other vaccines are in development globally and, with contenders from the UK, the US and China, the race to become the first universally recognized barrier against covid-19, which has claimed the lives of more than 788,000 people and has infected more than 22 million, it is still standing.

METRO DE PANAMÁ WILL EXPAND ITS SERVICE A FEW HOURS STARTING MONDAY

The general director of the Panama Metro, Héctor Ortega announced that after the reopening of new economic activities authorized this week by the health authorities and the flexibility that will be made from next Monday, August 24, of mobility restrictions, they have taken measures to offer better service.

Ortega explained that from this Thursday the Pan de Azúcar stations on Line 1 and San Antonio on Line 2 were reopened, as part of the expansion of the service; In addition, the biosecurity measures in the Metro have been reinforced and for next Saturday they will reactivate the service from 7:00 a.m. until 7:00 p.m.

“We are preparing for Monday and among the new measures that we take in the operational part was the extension of the schedule by one more hour, from 5:00 in the morning to 9:00 at night from Monday to Friday and the opening of Don Bosco station,” said Ortega.

The director pointed out that the Metro, due to the quarantine imposed by the authorities, suffered economic losses, given the low influx of users, it was decided to operate with 21 stations of the 31 that the Metro has, and they also reduced the number of trains to mitigate losses.

Ortega highlighted that in the first days of the pandemic the influx of users fell from 400 thousand users to 32 thousand; Currently and with the new reopening, they transfer 105 thousand users.

Among the biosecurity measures they took since the beginning of the pandemic and with which they will continue are the constant fogging of the wagons, placing signs to guide users to maintain their distance; placing alcohol-based gel devices and encouraging the use of masks.

STUDY REVEALS A POSSIBLE ‘WEAK POINT’ OF COVID-19

The Covid-19 virus.

An analysis of the differences between the coronavirus that caused the 2003 SARS epidemic and the one that caused the COVID-19 disease, apparently has found a weak point with which the virus can be attacked before it enters the body.

This was one of the findings made by the Mexican physicist and doctor Mónica Olvera and her team at Northwestern University (United States), who determined after a study that SARS-CoV-2 enters the body through its proteins Yes, they make contact with the angiotensin converting enzyme (ACE2) of human cells. In addition, he detected in conjunction with his colleague from Northwestern University Baofu Qiao that SARSCoV-2 has connections with positive charges in the spike protein (or spike) of its crown that can be blocked, as explained by the specialist to the BBC News .

According to the specialist, “they are blocking the virus”, by supporting how their experiment at the molecular level has initially reduced by 30% the connection of the pathogen with human receptors.

Olivera considered that the virus could be attacked before entering the body so that it no longer has so much attractive energy, and that it is not capable of infecting and entering.

Now the specialist hopes that, in three months, she can design a chemical compound that will triple the effectiveness of the blockade and that this will become a way to protect the body from a virus as contagious as SARS-CoV-2.

The publication of this research was made two weeks ago in ACS Nano, a specialized journal of the American Chemical Society.

RT OF COVID-19 IN PANAMA DOWN TO 0.99

Dr. Israel Cedeño, director of the Metropolitan Health Region reported this Tuesday, August 18, that the Rt or effective reproductive index of COVID-19 in Panama fell to 0.99.

The effective reproductive number of COVID-19 in Panama drops to 0.99. Telemeter Reports

Cedeño recalled that the Rt last week was at 1.11.

This means, explained the doctor, that each person who becomes infected with this virus can infect a maximum of one person or possibly infect no one.

The province of Panama continues to mark the highest number of new and active cases, followed by Panama Oeste, Chiriquí, Colón, Veraguas, Bocas del Toro, Coclé and the comarcas, while the province of Los Santos marks the least number of cases.

He explained that from March 9 to the epidemiological week that ended August 15, there was an increase in cases with the opening of block 2 of economic activities, but in the week of July 27, a decrease was reported, and there has been maintained the trend of confirmed daily cases.

Brett’s Comments

When this all started, I had no idea what an “Rt” was and mistakenly thought that it was a fixed number relating to the effective reproductive index.  Well, it’s not.  It behaves and adjusts in tune with our behavior.  I still have no idea how they calculate the ongoing rise and fall of the Rt, but what I do know is that all it takes is for the population to get too comfortable and we’re certain to push that Rt right back above 1.0 or 2.0 quicker than it takes for you to sneeze.

USE OF HYDROXYCHLOROCINE IN COVID-19 PATIENTS DIVIDES THE OPINION OF PHYSICIANS

The early use of hydroxychloroquine in COVID-19 patients is an issue that divides the opinions of doctors in Panama and the world. Recently, a group of Panamanian doctors requested the Government to use this drug early in COVID-19 patients.

Opinions on this issue are different. For Dr. Javier Nieto, hydroxychloroquine is an excellent drug for patients with autoimmune and rheumatological diseases and those suffering from malaria. However, he said that for COVID-19, to date, “there is no robust evidence to show that this drug administered early changes the clinical course of patients, the severity of symptoms, or in those patients with mild symptoms, nor in those patients with moderate symptoms”. Nieto argues that, although the use of the drug may be debatable or controversial, there are a large number of supranational organizations and entities that do not recommend it for COVID-19. “Evidence-based medicine goes beyond ideologies,” he stressed. After learning about flexibility in mobility, Dr. Nieto asked citizens to practice responsible self-care, wear a mask, wash their hands and practice physical distancing.

HOSPITAL SANTO TOMÁS ALREADY RECEIVES CONVOLVING PLASMA FOR THE TREATMENT OF COVID-19 PATIENTS

Dr. Marcel Penna Franco was the first to donate convalescent plasma to the hospital.

The Santo Tomás Hospital, since this week, joined the public hospitals that are receiving convalescent plasma to treat patients affected with Covid-19.

The head of the Blood Bank of the referred hospital, Heriberto Espino, explained this Tuesday, August 18, that people who suffered from Covid-19 and want to donate their plasma can approach the hospital at a time of 7:00 in the morning at 1:00 in the afternoon.

Espino indicated that they must carry their certification that they suffered from the disease in order to carry out the studies that will determine that they are suitable to donate, that is, they still have a sufficient amount of antibodies to the disease (proteins) present in their blood.

In fact, he indicated that of the patients who approached to donate, until now, there was one who could not do so because his protein (antibody) level had dropped, which regularly occurs when the SARS-CoV-2 infection was not severe.

Convalescent plasma is one of the approved therapies to treat Covid-19 patients, which has had satisfactory results.

The head of Mental Health at Hospital Santo Tomás, Marcel Penna Franco, was the first convalescent plasma donor within the hospital, after having undergone the disease approximately two months ago.

Penna Franco invited the recovered population to approach the hospitals to donate, because if each of the 55,845 became donors, many people affected with the disease would be able to be treated with this care therapy.

END NOTES

I’m a tough guy, but I’m also a sensitive guy.  After losing my step-Dad this past weekend, I also noticed how a few of my friends have lost some of their family members as well.  It’s very touching.  Our fluffies and furies are no different to some of us and losing one is an absolute heart jerker.  As new friend Cathy wrote me recently after losing her buddy “Tequila”recently; “Those wonderful 4-legged creatures take a big space in your heart”.  She sent this touching ballad with a video dedicated to our fluffies.  (I’m not crying, YOU’RE crying!)

Furry tribute

Stay safe…  Stay healthy…  And by all means (unless you’re adopting a stray from the local pound after seeing the END NOTES video) STAY HOME!!…
