Dogs Driving Me Bananas; Telemedicine for Covid-19 patients; Possibility of increasing hours of circulation is evaluated; Government approves payment of thirteenth-month bond; Our birds are Thriving even in this rain

Friday, August 14, 2020

The rains today were spectacular.  Mixed with the symphonies of thunder and the hum as the rain abused our tin roof, I had a very therapeutic morning and afternoon.  Then strumming through some of the social media chatter I saw how Fran Hogan, from franhoganphotography.com mentioned how she’s going to buy a banana with the money she’s made from a recent payout for her work.  She also added poetic expression with impeccable locution about her computer, but that’s irrelevant.  Recently, good friend Chuck stopped by and showered us with some small bananas he had from his back yard.  I think it was Eric Jackson who also talked about the fruit guy who passed by selling small bananas.  And if all of that weren’t enough to steer me on the path to bananas, I heard a commotion outside my office door just past noon.  Sure enough, Queen Sayira had given some of the bananas to the birds and they were having an absolute feast, even in all of this rain.

So, I made Banana Bread!

Banana bread is the easiest type of bread to make.  It’s almost impossible to screw it up.  Prep time is 10 minutes, cooking time is an hour.  Then BOOM, Banana Bread! Hell, I can recite the recipe by memory still, cream ½ cup of butter with ¾ cups of sugar, add two eggs, a little vanilla, 3 large mashed bananas (6 small ones), then add he dry mix of 1 ½ flour, ½ Baking Soda, ¼ salt.  Fold in ½ cup of walnuts and if you want ½ cups of raisins.  Smear butter on the inside bottom and sides of a bread pan, a little bit of flour so the loaf doesn’t stick and dump in the mixture. Set oven to 350 degrees (F), place the bread pan in the oven and wait.  Tell Alexa to set a timer for 55 minutes and there you have Banana Bread.

Step 1!!

The next step, apparently, is to let it cool for 15 minutes on a wire rack, cut four pieces for you and your family, take said plates of delicious Banana Bread to your family, come back to the kitchen just in time to see your dog Mickey eating the remaining loaf off the floor, proceed to yell at said dog chasing him out the door, pick up half-eaten-slobbery Banana Bread from the floor and then in a fit of rage, throw said chunk of gooey nastiness 35 yards out the kitchen door.  Professional Chef Tip:  Yelling “FFfffffff$%#&”&$&%&$&#&ck”, seems to help the bread cool off a bit more as well.

Step 2!

The final step to enjoying delicious, moist Banana Bread is to repeat all of the steps mentioned above between the lines “I can recite the recipe” through “and there you have Banana Bread”, ensuring however that upon removing from the oven, the bread is kept at human chest level, preferably on top of the refrigerator.

I love my dogs, I love my dogs, I love my dogs… but the big one is NOT my favorite right now.

Yoli’s Summary

COVID-19 patients who are morbidly obese are 60 percent more likely to die or require intubation, compared to people of normal weight.

If you are under 65 and obese, COVID-19 poses a special danger to you. A new report reports that the more obese you are, the more likely it is that you will die from infection with the new coronavirus or require mechanical ventilation to survive.

COVID-19 patients who are morbidly obese are 60 percent more likely to die or require intubation, compared to people of normal weight.

Obesity makes COVID-19 more likely to be serious, but age matters.

Mildly obese patients were 10 percent more likely to die or need a breathing machine, while those who were moderately obese were 30 percent more likely, according to the study.

But obesity posed a risk only in people younger than 65, the researchers added.

Anderson said this is likely because older people are sicker, more fragile and more likely to develop respiratory failure from COVID-19, due to health problems that are not related to their weight.

She and her colleagues reviewed the medical records of nearly 2,500 patients hospitalized for COVID-19 in March and April at Columbia University Irving Medical Center. They looked at body mass index (BMI), which is based on height and weight. Obesity begins with a BMI of 30, and morbid obesity is from 40.

About 25 percent of the patients died, and 22 percent required mechanical ventilation. On average, they were hospitalized for a week, the researchers found.

By looking at BMI, the researchers found that being overweight increased the patients’ risk of severe COVID-19, but only if they were young or middle-aged.

Anderson said there are a number of reasons obesity could make COVID-19 symptoms worse, starting simply with body mechanics.

Additionally, other diseases associated with obesity, such as diabetes or hypertension, may contribute to the impact of COVID-19 on the body, said Dr. Aaron Glatt, chief of infectious diseases at Mount Sinai South Nassau in Hewlett, New York.

Based on the evidence, obese people may need to be even tougher about social distancing and other methods of protecting themselves from infection, Anderson stressed.

It’s important for doctors and patients to really think about how long to prolong social distancing, whether to keep trying to avoid contact until the pandemic is better controlled.

People who are obese and young may need to consider prolonged social distancing and minimizing risks for a little longer than people who are not obese.

SHOPPING CENTERS MUST CONTROL CUSTOMER’S ENTRY FOR PURCHASE WITHDRAWALS

The Minsa authorized the reopening of beauty salons, retail sales, car sales and construction projects.

After the Ministry of Health of Panama (Minsa) announced the reactivation of retail stores starting next Monday, August 17, the Vice Minister of Internal Commerce, Omar Montilla clarified that the premises in shopping centers will open, but it is not it will allow the movement of people in the facilities.

Companies with reactivated activities will be able to process safe conducts from this Thursday. Atenógenes Rodríguez

Montilla explained that the retail sale will be reactivated through online sales, and the owners of premises and the security team of shopping centers must maintain control so that customers who are going to withdraw their purchases do not enter.

“They will be able to open (shopping centers) but not for people to circulate as usual, where they were going to do shopping tourism. They are simply going to specifically withdraw to the warehouse where they are going to make their purchase and they must withdraw, or that the warehouse give them the opportunity to take their product to the parking lot “, he clarified.

According to the Minsa, retail stores may deliver goods at the doors of their premises and in parking lots only to those customers who place their orders online, complying with biosecurity protocols.

Regarding beauty salons and barber shops, another of the activities reactivated by the Minsa, Montilla reiterated that these will be able to operate through appointments and with a capacity of 50%.

He indicated that these companies must manage the safe-conduct for their collaborators from this Thursday, August 13 through the page https://validacion.salvoconductos.mici.gob.pa/register.

The vice minister said that they will be conducting inspections in conjunction with the Ministry of Labor and Labor Development (Mitradel) and the Ministry of Health, to verify compliance with the protocols.

BEFORE THE ECONOMIC REOPENING, MEDICAL UNIONS REQUEST THAT PATIENT BEDS BE GUARANTEED

The country’s medical unions requested a meeting with the President of the Republic, Laurentino Cortizo, to inform him “first-hand” of the “real” conditions of the public health system and the urgent measures to get out of the health collapse” as a result of the COVID-19.

They stated that, although economic development is necessary for the support of the individual, the main right that must be guaranteed is life and consequently health, so the actions to face the pandemic must be “responsible and wise”.

The statement, which was agreed after a meeting held on Wednesday, August 12, has the endorsement of the Association of resident and intern doctors of the Social Security Fund, the Association of specialist doctors of the Santo Tomás Hospital, the Association of doctors, dentists and related professionals from the Social Security Fund, the Panamanian Society of General Medicine and the Panamanian Society of Public Health.

The representatives of these unions also asked the Government to implement measures to regain control of the pandemic, such as: strict isolation outside the home with medical care and safe food support for affected families; the urgent supply of sufficient and quality protective equipment for health personnel; provide sufficient swab and serological tests so that they are not denied to people; make the massive distribution of surgical masks or at least three correct layers to vulnerable families and access to permanent water and food for the communities.

“We demand that the Government eliminate the current badly done quarantine measure and that an effective total quarantine be implemented, with exclusive safe-conducts for essential personnel” and then move on to the implementation of effective epidemiological fences in areas of high transmissibility, were others of the requests.

They estimated that, before starting the economic reopening, sufficient isolation, hospitalization and intensive beds be guaranteed – at least by health region – as well as the effective appointment of the necessary health personnel. In addition to this, the comprehensive supply of all hospital drugs and early primary care management must be guaranteed to improve Covid-19 survival.

This week the Ministry of Health (Minsa) announced that, starting next Monday, August 17, beauty salons and barber shops will be able to serve clients, by appointment, with screens between each cubicle and without exceeding 50% of the capacity of the premises.

Likewise, the sale of vehicles and the opening of retail stores will be allowed. In the latter case, customers must collect the merchandise in the parking lot or at the door of the establishment.

It will also allow the resumption of public and private construction works, with “significant progress in execution.”

TELEMEDICINE PILOT PLAN ADVANCES IN JUAN DÍAZ FOR THOSE INFECTED WITH WITH COVID-19

In the township of Juan Díaz, a telemedicine pilot plan is being carried out to provide remote care to positive patients with COVID-19. In one week, about 200 cases were monitored to avoid health complications.

In a call center, TVN Noticias found doctors participating in the Telemedicine Pilot Plan, which is currently being carried out in the Juan Díaz district, in full swing.

Since last Monday, the United for Panama Foundation, in the hands of the Ministry of Health, feeds on the database of the Juan Díaz Policenter, on patients who are daily diagnosed positive with COVID-19. A whole medical staff to monitor.

“It is made up of nurses. It is made up of respiratory therapists, we also have nursing assistants, and we also have a team of doctors. In addition to the doctors that we have in the call center, we also have a support group, who are doctors, and within that support group we have specialists such as obstetricians, internists, pediatricians, psychologists and nutritionists, ”explained Jair Chen, Director of the United for Panama Foundation, made up of a group of companies in charge of the pilot plan, to support the State in this health crisis.

There is also a rapid response team to offer respiratory therapies at the patients’ homes.

Daily they have data of about 15 new positives in COVID-19, which they monitor up to every 4 hours. “We have contemplated that, for the care of these patients in this program, we will be able to reach a peak of approximately 3,600 to 3,800 patients, which we will be able to attend to in 3 months.

Because let us remember that this home care plan is a pilot plan, it is a pilot plan that we are going to attend to those three months, and based on those results, it will allow us to see the viability of being able to scale to other townships ”, explained the director of the United for Panama Foundation. As explained, patients with COVID are given a kit through the Home Care Plan, which, among other things, contains an oximeter and thermometer, so that when they are called, they offer the measurement data. The idea is that patients do not have to leave their homes. With the support of the Todo Panamá Movement, they are given bags with food.

WORLD HEALTH ORGANIZATION SAYS UNLIKELY THAT THE CORONAVIRUS CAN BE TRANSMITTED THROUGH FOOD

The World Health Organization (WHO) assured this Thursday that a transmission of the new coronavirus through food should not be feared, after China announced that it had discovered covid-19 particles in imported food.

“We do not believe that the coronavirus can be transmitted through food. If we have understood it correctly, China looked for the virus in packages, checked it with hundreds of thousands and only found it in very few, less than ten tested positive,” explained the scientist Maria Van Kerkhove, Head of the Emerging Diseases Unit at WHO.

However, “we know that it can remain on the surface for a while” and “we have given directions, through the FAO (the Food and Agriculture Organization of the United Nations)” so that workers in the food sector are in safety in your work environment, Van Kerkhove said.

Chinese authorities announced on Thursday that they had found particles of the virus during a routine check in frozen chicken wings from Brazil and in shrimp wrappers imported from Ecuador.

“Our food with regard to covid is safe,” defended Michael Ryan, director of health emergencies at the WHO, who recalled that “there is no proof that food or food chains are involved in the transmission of the virus.”

“This type of information should not be exaggerated,” Ryan said. “People are scared enough of the pandemic already,” he added.

RESEARCH PROPOSAL DEVELOPED TO IDENTIFY ASYMPTOMATIC CASES OF COVID-19

People clean their hands with alcohol gel.

A group of Panamanian researchers developed a research proposal with a view to applying to the Call for Rapid Response to COVID-19 in Panama, from the National Secretariat of Science and Technology (Senacyt). The investigation to be carried out by this group will evaluate the presence of SARS-CoV-2 by means of “combined diagnosis of qRT-PCR and rapid test of IgG and IgM by immunochromatography”, in the asymptomatic population with mild symptoms. This study will evaluate and diagnose those individuals who present symptoms other than fever, dry cough and respiratory distress and who, because they do not present such symptoms, were not considered for a diagnostic test reference. In addition, it will allow the generation of an accelerated diagnostic screening algorithm for COVID-19, based on the characteristics that people suspected of the disease are presenting. This research promoted by Senacyt, will begin with a sample of 2,500 people in the Biomedicine laboratories of the Inter-American University of Panama and its results will later be shared with the Ministry of Health (Minsa). The group is made up of scientists from the Gorgas Memorial Institute, the Interamerican University of Panama, the Latin University of Panama and the American University of Panama.

ITALY PROHIBITS THE ENTRY OF PEOPLE WHO HAVE STAYED MORE THAN TWO WEEKS IN PANAMA

Anyone, even asymptomatic, must inform the local health authorities of the date of their entry into Italy.

Italy prohibited entry to people from Colombia for fear of new coronavirus infections, official sources indicated this Thursday.

The decision was made by the Minister of Health, Roberto Speranza, who also announced that people arriving from Croatia, Greece, Malta and Spain must necessarily undergo the coronavirus test.

Colombia was added to the list of countries considered at risk, after having exceeded 12,000 deaths on August 8.

“We must follow the line of caution to defend the results obtained in recent months thanks to everyone’s sacrifice,” explained the Italian Health Minister in a statement.

Travelers entering the peninsula from the countries indicated by an airport, port or border post can undergo rapid tests in situ, present a certificate obtained in the last 72 hours or take the test in the following two days after respecting a brief quarantine.

Anyone, even asymptomatic, must inform the local health authorities of the date of their entry into Italy.

Italy had already closed its doors to travelers from Kosovo, Serbia, Montenegro, Bosnia, North Macedonia and Moldova. Those arriving from Bulgaria and Romania must observe quarantine and medical controls.

Italy also prohibits entry to people who have stayed more than two weeks in Armenia, Bahrain, Bangladesh, Brazil, Chile, Kuwait, Oman, Panama, Peru and the Dominican Republic.

Several regions of Italy decided at the beginning of the week to impose a quarantine on travelers coming from European countries considered at risk, due to the increase in infections among Italians who have spent holidays in them, in particular in Greece and Spain.

PCR AND QUARANTINE TEST TO ENTER PANAMA

The announcement made by the Ministry of Health (Minsa) on the measures that Panamanians and foreigners who enter the country on connecting flights authorized by the Government must comply with generated doubts in the Panamanian air sector.

On Tuesday, August 11, the head of the Minsa, Luis Francisco Sucre, announced that nationals and residents will have to undergo a PCR test 48 hours before the flight that will bring them to Panama.

The official indicated that it will also be mandatory that they remain in mandatory quarantine for 14 days after their arrival in the country. In addition, upon arrival at the Tocumen International Airport, they will have to sign a document authorizing their inclusion in the monitoring program that the health authorities keep active to determine the evolution of active cases of the coronavirus.

On July 31, Decree 300 was published in the Official Gazette authorizing the operation of a “mini hub of connections” in Tocumen. The decree establishes that Panamanians and residents may leave the country on any of the connecting flights that depart from Tocumen. It also allows passengers to arrive at the airport from other destinations to board the air connections that leave Panama, but with the condition that they do not stay more than 6 hours in Tocumen.

The airlines consulted said they are waiting for the Ministry to publish the decree that explains how they should apply the conditions announced by Sucre last Tuesday.

Spokesmen for at least 3 airlines commented that they do not know if the airlines will be responsible for demanding the documentation of the PCR test from passengers and they also do not have information on when they should request this data.

“We do not know if people must present the proof when buying the ticket or when boarding the plane to Panama,” they indicated.

At least four airlines have confirmed that they will have flights to and from Panama: Copa Airlines, AirFrance, KLM and United. This number is expected to increase in the coming days.

MINSA EVALUATES THE POSSIBILITY OF EXTENDING FREEDOM FROM 2 TO 4 HOURS DURING THE MOBILITY RESTRICTION

Beauty salons and barber shops may reopen their activities and receive clients by appointment, without exceeding 50% capacity.

Minister Luis Francisco Sucre during his intervention in the Assembly’s Budget Committee indicated, among several issues, that the exit time of the ladies could be increased after the criticism generated by maintaining the curfew and departure by schedule and gender.

“We are evaluating the possibility of increasing it from 2 to 4 hours so that each one can run the errands they need. Our ladies with 4 hours if they are going to have time to do everything that corresponds, but it is part of what is in planning, “said the health minister.

He clarified as health authorities we have to maintain the time that corresponds to them to go out and urged people to have the procedures done by part if it is about the attention in beauty salons.

“If we free up the time we are going to cause crowds in different places and this is gradual, later if the figures continue to drop they will be granted 4 hours so that our Panamanian ladies can make all the arrangements they need,” he said.

Many of the criticisms that were given on social networks by women who commented that the processes and beauty treatment do not last two hours. It is very little time to go to see you.

So far, Minister Sucre stressed that it is not possible to lift the quarantine, the measure will continue in the sectors where it has been implemented and stressed that everything will depend on the behavior of the population and the reproductive index (RT) of the coronavirus.

It should be noted that the provinces of Panama and West Panama continue with the restriction of mobility by gender and ID number in addition to the curfew from Monday to Friday from 7:00 pm.

Women must go out Monday, Wednesday and Friday, while men only Tuesday and Thursday both depending on the last ID number.

As of Monday, August 17, beauty salons and barbershops will be able to reopen their activities and receive clients by appointment, without exceeding 50% capacity.

These businesses must have partitions between the cubicles that separate customers and the use of masks will be mandatory. There may not be crowds either inside or outside the beauty salon or barbershops.

COMPANIES VALUE REOPENING, BUT CALL FOR GREATER MOBILITY

Union representatives described the restart of certain activities as of next Monday as a positive step, but warned that it would be insufficient for businesses to be sustainable, since mobility restrictions remain.

The president of the Chamber of Commerce, Industries and Agriculture, Jean-Pierre Leignadier, said that “in the coming days we have to make adjustments, talking with the authorities about some measures that negatively impact the ability to do business in a sustained manner, when there are people that they cannot go out some days and others yes”.

The president of the Panamanian Association of Shopping Centers, Nadkyi Duque, said that the sale through digital channels and withdrawal in the store is a positive step, but it will not generate enough volume, although she valued it as a way to demonstrate that the activity is safe . Beauty salons, car sales, oenegés and certain works will also restart.

In addition, the Ministry of Education received the go-ahead to start with 9 of 58 projects for improvements in schools.

PFIZER AND BIONTECH VACCINE SHOWS ROBUST IMMUNE RESPONSE

Logo of the American company Pfizer at the company’s world headquarters in New York.

The RNA vaccine candidate developed by Pfizer and BioNTech has been shown to induce a robust immune response in a preliminary clinical trial conducted with 45 healthy adults ages 18 to 55, the results of which are published today in Nature.

BNT162b1 is the most promising of the four vaccines developed by the American pharmaceutical company Pfizer and the German company BioNTech, all based on messenger RNA to elicit an immune response.

These types of vaccine candidates are generally considered safe and have facilitated the rapid development of vaccines against SARS-CoV-2.

According to interim data from the phase 1/2 clinical study, conducted to evaluate the safety, tolerability, and immunogenicity of the vaccine, BNT162b1, which is administered intramuscularly, was tested in 45 healthy adults (23 men and 22 non-pregnant women; 37 participants were Caucasian) aged 18 to 55 years.

Candidates – randomly assigned – received 10 micrograms (µg), 30 µg or 100 µg of BNT162-1, or a placebo; in addition, participants in the 10µg and 30µg groups also received a second dose on day 21.

The authors found that BNT162b1 was generally well tolerated, although some participants experienced mild to moderate side effects within seven days of vaccination, which increased with dose level.

The most common adverse reactions were local pain at the injection site, fatigue, headache, muscle pain, chills, fever, and sleep disturbances.

However, the vaccine elicited a robust immune response in the participants, which increased with dose level and with a second dose.

Antibodies against SARS-CoV-2 remained 21 days after the first vaccination at all dose levels, and a substantial increase in neutralizing antibodies against SARS-CoV-2 was recorded seven days after the second was administered. 10-µg or 30-µg doses.

The immune response was much stronger in the 30-µg group than in the 10-µg group.

However, there were no notable differences in immune response between the 30-µg and 100-µg groups after the first dose, and since the participants who received the 100-µg dose also experienced greater side effects, they did not receive a second dose. .

Participants’ neutralizing antibody levels were 1.9 to 4.6 times higher than those of patients recovering from SARS-CoV-2 infection.

However, while this comparison provides a benchmark for evaluating the immune response elicited by the vaccine and its potential to provide protection, phase 3 trials are needed to determine the efficacy of BNT162-1.

Adults aged 65 to 85 years are being recruited for this study, and in later phases, priority will be given to recruiting more diverse populations, the study notes.

Stay safe…  Stay healthy…  And by all means (unless you overdid it with the Banana Bread and want to share with others) STAY HOME!!…
